For each question, you can answer 0-not at all; 1-several days; 2-more than half the days; 3-nearly everyday.
1. Feeling nervous, anxious, or on edge?
2. Not being able to stop or control worrying?
3. Worry too much about different things?
4. Difficulty relaxing?
5. Being so restless, it is difficult to sit still?
6. Becoming easily annoyed or irritated?
7. Feeling afraid, as if something awful might happen?
Scoring: 5-9 = mild anxiety symptoms; 10-14= moderate anxiety symptoms; 15-21 = sever anxiety symptoms
